The Significance of Lung Function Changes in Asbestosis

چکیده

Asbestos is the name given to a series of minerals composed of fibrous silicates of magnesium and iron which find widespread use in industry. Inhalation of the dust may cause asbestosis, a crippling form of pulmonary fibrosis, which usually does not appear until many years after the initial exposure. There is also an increased incidence of carcinoma of the lung in patients with asbestosis, and Doll (1955) estimated that the risk of lung cancer in asbestos workers was approximately 10 times greater than in the general population. The diagnosis of asbestosis at present depends on the history of exposure to the dust together with certain physical signs and radiological changes. The most characteristic features of the latter are a " ground-glass " mottling of the lower zones of the lung fields with a shaggy border to the heart. The physical signs are clubbing of the fingers and fine metallic crepitations on auscultation of the lung bases. Unfortunately many cases even with definite physical signs do not show the characteristic radiological changes, and the early diagnosis of the disease may be most difficult. The presence of "'asbestosis bodies" in the sputum simply proves that asbestos dust has been inhaled and does not necessarily imply that fibrosis is present. The purpose of the present work was to relate the changes in lung function in asbestosis to the clinical and radiological signs of the disease and to try and determine the value of lung function tests in assisting with its early diagnosis.

The Radiological Diagnosis of Asbestosis

The recognition of different pneumoconioses is usually based on a history of exposure to a given dust together with the presence of characteristic changes in the chest radiograph. This implies that the radiological appearances are reasonably specific. Moreover, if they are to be used not only for clinical diagnosis but also for compensation purposes, and for the study of the disease in relation...

Serial pulmonary function tests in patients with asbestosis.

Serial lung function tests were performed on 17 patients with asbestosis. A marked fall in the transfer factor often preceded any significant decline in the vital capacity. Changes in vital capacity and transfer factor did not appear to give any indication of the prognosis in these patients. Death was more commonly due to carcinoma of the lung than to the effects of the lung fibrosis.

Lung function changes in pleural asbestosis.

The aim of this study was to examine the relationship between radiographically detectable pleural changes and lung function in pleural asbestosis. One hundred and twenty chrysotile asbestos-exposed workers were enrolled in this retrospective study. For each examinee the length of asbestos exposure and the degree of dust cover at the workplace were assessed as well as the radiological and functi...

Asbestos related pleuropulmonary lesions and the erythrocyte sedimentation rate.

Asbestos related lesions of the lung parenchyma and the pleura can be divided into three main types: parietal pleural plaques, diffuse interstitial fibrosis ("classical asbestosis"), and a third type of reaction affecting both the pleura and the lung parenchyma. The last type includes such lesions as acute pleurisy, diffuse pleural thickening, and rounded atelectasis. Among 1344 patients with a...
